Please read this form and check the required box on the Registration form

I have read all registration materials provided to me either in handouts or on the OLY Website and realize that I am responsible for all policy that is contained in those materials.  This information includes, but is not limited to:  Registration instructions, payment options and requirements, due dates, refund policies, emergency information, code of conduct information, volunteer information, grocery coupon program, and kick-off information.

I further understand that OLY makes available most information by way of email and the OLY Website, backing up this information with handouts and verbal coach's instructions when needed.  I am familiar with the Website at www.olyswim.com and know that I can get help using it when needed. 

By checking the box on the OLY online registration form I agree that I have read this waiver and its contents.
