Please read this form in
its entirety. If you register your
swimmer with OLY, it means that you have read and
understand this document.
I have read all registration
materials provided to me either in handouts or on the OLY Website
and realize that I am responsible for all policy that is contained
in those materials. This information includes, but is not
limited to: Registration instructions, payment requirements,
due dates, refund policies, emergency information, code of conduct
information, volunteer information, grocery/scrip program, and
kick-off information.
I further understand that OLY makes available most information by
way of email and the OLY Website, backing up this information with
handouts and verbal coach’s instructions when needed. I
am familiar with the Website at www.olyswim.com and know that I
can get help using it when needed.
By registering my swimmer with OLY, I
agree that I have read this waiver and its contents.
